It is dated to September 22, 1944 and signed in blue ink by Major General and Commander of the 3rd Mountain Division, Paul Klatt (1896–1973), a recipient of the Knight’s Cross with Oak Leaves.

The document is dated to Piešťany (western Slovakia) on December 16, 1944 and signed in blue crayon by a Stabsarzt (Staff Doctor) and Head Physician of medical hospital 2/527 (R), the name is indecipherable.
